Mini Book For Mom

This little book is filled with 11 pages of prompts for you to answer about your mom and comes with coordinating bindings.

Here’s what you’ll need to make your own Mini Book for Mom:

Directions for Mini Book for Mom

Step 1:

Print out the booklet.

Step 2:

Answer the questions.

Step 3:

Cut out the individual pages.

Step 4:

Cut out a binding from the first page and fold in half.

Step 5:

Stack pages in order, with the title page on top. Insert the pages into the binding and staple together.

Mother’s Day Questionnaire

Our questionnaire gets the craziest answers from kids! It comes in different color ways, so your kids can pick the one they want and with names for Mom, Grandma, Granny, Mimi, and Nana so no matter what you call her you are covered.

Supplies:

  • Printable
  • Scissors
  • Glue stick
  • Cardstock
  • Pencil

Directions for Mother’s Day Questionnaire

Step 1:

Select the color and title you prefer and print onto a matte photo paper for best results.

Step 2:

Have your child answer the questions. Their handwriting and spelling are half of the gift!

Step 3:

Cut the cover and questionnaire out.

Step 4:

Fold a piece of colored cardstock in half.

Step 5:

Glue the cover on the front and the questionnaire on the inside.

  1. Super cute! I bought it and can’t wait to use it! I just have a question about what to use for the “binding” for the booklet – I can’t seem to find the page to print out, unless I’m just supposed to print one of the other Mother’s Day cards and just cut that up? Thank you!
